Radical Prostatectomy is the procedure to remove the entire prostate gland and tissues around it. This surgery is performed to treat prostate cancer.

Various techniques used to perform Radical Prostatectomy are:

  • Retropubic 
  • Perineal
  • Laparoscopic

Some doctors may perform the laparoscopic surgery using robotic arms controlled via console. 

Indication

Radical Prostatectomy is perfomed to treat localized prostate cancer, and disallowing it to spread beyond the prostate gland.

Advanced prostate cancer cannot be cured by prostate removal.

Pre-Procedure

Before surgery, your doctor will conduct some tests to check your bladder and urethra.

You may be asked not to eat or drink a few hours before the procedure. In some cases, a laxative will be a day before the surgery to clean the bowels.

Blood thinning medication has to be stopped atleast a week before the surgery.

During-Procedure

Radical Prostatectomy is performed under general anesthesia and lasts about 3 to 4 hours depending on the type of technique used by your doctor.

  • Retropubic Surgery: An incision is made below the belly button to access the pubic bone. Prostate gland and tissues around it are removed. 
  • Perineal Surgery: It is also an open surgery. Comparatively a smaller incision is made between anus and scrotum.
  • Laparoscopic Surgery: Small incisions are made in the belly to insert the laparoscope. Using specialized instruments through the laparoscope, the prostate is accessed and removed.
  • In most cases, the seminal vesicles are also removed.
  • After removing the prostate gland, the urethra is attached to the bladder neck so as to allow flow of urine.
  • After the procedure, a tube will be placed in your bladder for a few hours to drain the urine. 

Your surgeon may also remove the lymph nodes to test them for cancer. 

Post-Procedure

The patients can go back home in 1 to 4 days, depending on the medical condition and the technique used for the surgery.

Catheter will stay inserted in bladder for 7 to 15 days, to drain the urine.

Your doctor will recommend the following

  • Walking and leg exercises to prevent clotting of blood
  • Biopsy after a week to check if there is any recurrance of cancer

  • Heavy blood loss during the surgery
  • Infection in the urinary tract
  • Erectile dysfunction
  • Difficulty in controlling the urine
